Logos is IN NETWORK and accepts the following insurance:
**We are a TRICARE Certified Non-network provider.**
* TRICARE Select beneficiaries do not require a referral for treatment.
* All TRICARE Prime beneficiaries will need to obtain a referral from their PCP to be treated due to our Non-network status.
-TRICARE Prime members may alternately choose to pay up-front for services rendered and file their own claims.

If you do not see your insurance plan listed above don't fret, consider the Out of Network Insurance Reimbursement Option.
OUT OF NETWORK HEALTH INSURANCE REIMBURSEMENT
If we do not accept your insurance plan, but you have a commercial insurance plan (not Medicare), you may be able to obtain reimbursement for your out-of-pocket expenses.
PRIVATE SELF-PAY
Sessions are offered through private pay to allow you the freedom to have counseling your way, and not the way your insurance company prefers.
Logos also accepts Flexible Spending Account (FSA) and Health Savings Account (HSA) as a form of payment.
FEES (Not Covered By Insurance)
* Late cancellations/Missed Appointment – fewer than 24 hrs. prior to appointment $30.00
* Non-sufficient funds (bounced) check $25.00
* Past-due accounts – over 30 days $25.00 per month
* Checks returned due to insufficient funds will incur a fee of $45.00
“Clients are strongly discouraged from pursuing the deposition or subpoena of their counselor or therapist. Although you bear responsibility for the associated testimony fee, it is crucial to acknowledge that the provider's testimony may not exclusively support your position. Providers are obligated to testify strictly to the factual aspects of the case and offer professional opinions, adhering to ethical and legal guidelines.” For those who fail to heed this discouragement, we do charge a minimum court subpoena/appearance fee of $1,000.00 per appearance (in-person or via video).

$2.00 per page for pages 1-10
$1.00 per page for pages 11-20
$0.90 per page for pages 21-60
$0.50 per page for pages 61 and above
The Above Fees Do Not Include Postage if Mailed.
